Hospital case managers & discharge planners

A discharge is coming and the family has no plan for the first nights home. When you call, a real person answers. We present experienced caregivers the family can engage directly — often the same day — and we stay reachable while they choose.

Social workers at rehab facilities

When therapy ends and the family needs help at home to make the discharge stick, we present candidates matched to what the plan of care calls for. The family chooses; the caregiver works for them, not us.

Directors of nursing & wellness at senior communities

One-to-one attention inside independent living, assisted living or memory care, when a resident needs more than the community can provide. The family engages the caregiver directly; we help with the introductions and the paperwork.

Geriatric care managers & private nurses

You coordinate the care. The caregivers we refer work to their own level of competency under your direction, and we present candidates whose experience fits the plan — then stay reachable as it changes.

Guardians, trustees & elder law attorneys

We work alongside guardians, conservators, professional fiduciaries and care managers, and can help support a plan of care with the right caregiver — an extension of the care you coordinate, on a schedule your client sets. Weekly billing runs through a secure third-party billing platform and year-end tax forms are issued, which keeps the paperwork clean.

Placement advisors

When a family you are guiding chooses to stay home, or needs support while a move is decided, we present experienced caregivers they can engage directly — and we tell you honestly what we can and cannot do.

How the model works

Hallmark is a referral agency, not a home-care employer. The caregivers we refer work directly for the family, more of what the family pays reaches the caregiver, and the family receives higher continuity of care.
