Therapy is ending. The family needs help at home to make the discharge stick.
When a rehab stay ends and the plan of care calls for support at home, we present experienced caregivers matched to what it calls for. The family chooses; the caregiver works for them, not us.
Four steps, and the family stays in charge.
You call, or the family does
Tell us the discharge date, the hours, and what the plan calls for at home — transfers, mobility, meals, supervision, the routines your therapists set up.
We present candidates
Experienced caregivers whose availability and experience fit, with their background and experience.
The family chooses
The family interviews, decides, and engages the caregiver directly.
Care starts at home
The caregiver follows the family's direction — and works alongside home health when it is coming too.
Hallmark facilitates.
- Answer the phone — a real person, days, nights and weekends.
- Recruit and screen experienced caregivers, and present candidates whose experience and availability fit what you tell us.
- Help coordinate the details for the family — getting started, scheduling tools, weekly billing and the year-end tax forms — so those tasks stay off their plate.
- Help families arrange for a long-term care insurance policy to pay, including the carrier’s forms and invoices.
- Tell you honestly, and quickly, when we cannot help in time.
The family decides.
- Interviews and chooses the caregiver, and engages the caregiver directly — the caregiver works for the family, not us.
- Sets the schedule and the tasks, and directs the day-to-day care.
- Decides whether to continue — at any time, for any reason.
- Owns the plan of care, with the clinicians and professionals the family works with. We do not assess the patient or write the plan; the caregivers we refer work to their own competency under the direction of the family’s nurse or care manager.
In one breath.
Approvals and memberships we hold.
Answered plainly.
Can a caregiver start inside the facility before discharge?
Yes. Many families use us for one-to-one supplemental care inside a rehab setting, where staff cannot provide constant personal attention — and the same caregiver can continue at home.
What if the family also has home health coming?
Home health is short-term, physician-ordered, intermittent skilled care. Home care is ongoing help with daily life. The caregivers we refer work alongside home health, to their own level of competency, under the direction of the family's nurse or therapist.
Can care continue while the family decides on a longer-term move?
Yes — for days or months. The family engages the caregiver on a schedule they set and can end at any time, so nothing about it forecloses a later decision.

Hallmark is a referral agency, not a home care employer. The caregivers we refer work directly for the family, more of what the family pays reaches the caregiver, and the family receives higher continuity of care.
