What we do

Hallmark facilitates.

  • Answer the phone — a real person, days, nights and weekends.
  • Recruit and screen experienced caregivers, and present candidates whose experience and availability fit what you tell us.
  • Help coordinate the details for the family — getting started, scheduling tools, weekly billing and the year-end tax forms — so those tasks stay off their plate.
  • Help families arrange for a long-term care insurance policy to pay, including the carrier’s forms and invoices.
  • Tell you honestly, and quickly, when we cannot help in time.
What stays with the family — and with you

The family decides.

  • Interviews and chooses the caregiver, and engages the caregiver directly — the caregiver works for the family, not us.
  • Sets the schedule and the tasks, and directs the day-to-day care.
  • Decides whether to continue — at any time, for any reason.
  • Owns the plan of care, with the clinicians and professionals the family works with. We do not assess the patient or write the plan; the caregivers we refer work to their own competency under the direction of the family’s nurse or care manager.

How fast can a caregiver be in place after a discharge?

Often the same day. Families reaching us after a hospital discharge have had experienced caregivers in place within hours. Tell us the situation and we will be straight with you about what is possible.

Can the caregiver help with what the discharge instructions call for?

The caregivers we refer work to their own level of competency — many can support medication reminders and other everyday health tasks under the direction of the family's home health nurse or care manager. If the situation calls for a licensed nurse, tell us and we can present one. The family, and the clinicians they work with, own the plan of care.

What does it cost the hospital, and what does the family pay?

Nothing to the hospital, and there is no arrangement between Hallmark and any hospital. The family agrees on a rate with our fee already included and is billed weekly through a secure third-party platform.
