It depends on availability and is not something we can promise, but far fewer faces is exactly what the model is built for.

We answer the phone — a real person, days, nights and weekends — recruit and screen experienced caregivers, present candidates whose experience and availability fit what you tell us, and help coordinate getting started, scheduling tools, weekly billing and year-end tax forms. We also help families arrange for a long-term care insurance policy to pay, including the carrier’s forms and invoices.
Your family
You interview and choose the caregiver, and engage them directly — the caregiver works for you, not us. You set the schedule and the tasks, and direct the day-to-day care. You own the plan of care, with the clinicians you work with; we do not assess your loved one or write it. And you decide whether to continue — at any time, for any reason.

Can we arrange care before the discharge date?
Yes — and it helps. Call as soon as you know a discharge is coming, even if the date might move. We can present candidates ahead of time so you can interview and choose before your loved one comes home. If the date changes, tell us and we will present candidates for the new dates.
Can the caregiver help with what the discharge instructions call for?

What if home health is also coming?
Home health is short-term, physician-ordered, intermittent skilled care. Home care is ongoing help with daily life. The caregivers we refer work alongside home health, to their own level of competency, under the direction of your nurse or therapist — and they are there for the hours in between the visits.
What if we only need help for a few days?
That is common. You set the hours and the length. Families often start with overnights or around-the-clock help for the first stretch home and reduce hours as their loved one recovers — you can change or end the schedule at any time.
Can long-term care insurance pay for care after a hospital stay?
Often, yes. Hallmark Homecare is an Approved Provider with John Hancock for long-term care insurance, and many other long-term care policies reimburse care from a caregiver you engage through Hallmark. We take care of the provider side — our approved-provider status, the provider forms your carrier asks us for, and the paid invoices you submit; understanding your own policy and working with your carrier stay with you.
